Students initiating an appeal must complete this form and are expected to document any circumstances cited in the case.

• Company Sponsorship Agreement/Tuition Authorization Form

Students who are having tuition paid or reimbursed by their employer need to read, sign and take this form to their company's Personnel Officer. Have the Personnel Officer complete the form, sign and fax or return the completed form to the Cashier's Office.
