Revising Student Information
- It is very important for you to keep the College informed of any changes in contact information and/or education plans.
- If you change your name, address, telephone number or education major, then complete a Records Revision form to officially notify the College. Your record will then be updated in the College’s student database.
- The Records Revision form is available from the Student Records Office or as a printable PDF file.
- The completed Records Revision form can be either submitted directly to the Student Records Office, mailed to the Student Records Office at the address listed below, or faxed to the Student Records Office at 419-334-9035.
