Requesting Academic Transcripts

Academic transcripts are available to students by completing a written request with student signature authorizing the release of the information.

The transcript request form is available from the Student Records Office or as a printable PDF file.

The fee for each copy of an official transcript is $5, payable by cash, personal check or by credit card.

The completed transcript request form can be submitted either in person to the Student Records Office, or by mail to the address given below, or by fax at 419-334-9035. If faxed, please provide credit card information and expiration date on the request form so that fee payment can be made.

Allow up to two weeks for processing an academic transcript.
